Parental Permission and Release Statements & Code of Conduct

The following statements must be read by all parents/guardians prior to registering minors for Covenant on-campus and off-campus programs and events.

Medical Treatment Permission

I (we) authorize an adult, in whose care the minor has been entrusted, to consent to any emergency x-ray examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any physician or dentist licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital or emergency care facility. The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned children or students pursuant to this authorization.

General Policies

PERMISSION TO PARTICIPATE: The undersigned does hereby give permission for our (my) above-named children/students (“Participants”), to attend and participate fully in Covenant Church’s (“Church”) ministries, programs, events, and activities, including on-campus and off-campus  ministry trips.

LIABILITY RELEASE: In consideration of the Church allowing the Participants to participate in Church events, we (I), the undersigned, do hereby release, forever discharge and agree to hold harmless the Church, its directors, employees, volunteers and agents from any and all liability, claims or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participants while involved in the event. Furthermore, we (I), and on behalf of our (my) Participants, hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved with Church events therein.

INTERNAL PHOTO RELEASE: By participating in the Church’s programs, the undersigned gives permission for the Church to use photographs or video of my children/students in internal Church emails and/or photo slideshows during services. Please note that these slideshows are also included in the Church’s livestream, which is posted to social media.

PUBLIC PHOTO RELEASE: The undersigned gives permission for the Church to publish photographs or video of my children/students on the church website, social media, and/or other external church publications. If you would like to request an exception, please contact mperry@covenantpalmbay.org.

*TRANSPORTATION PERMISSION: The undersigned does also hereby give permission for Participants to ride in any vehicle driven by an approved adult chaperone while attending and participating in Church events. My children/students and I understand that SEAT BELTS SHALL BE WORN AT ALL TIMES during transportation and are responsible for compliance. Further, I understand that at no time shall an adult have only one child of the opposite gender in his/her vehicle, with the exception of emergency situations.

*EARLY RETURN HOME POLICY: Should it be necessary for our (my) child or student to be sent home from a Church event due to medical reasons, disciplinary action, or otherwise, the undersigned shall assume all transportation costs and responsibility.

*Applicable only for overnight or off-campus trips

Code of Conduct for Children/Students

Parents/guardians, please have your children/students read these rules and help them understand anything that may be unclear to them.

Children/students, you must agree to abide by the following statements:

As a participant in Covenant Church’s Family Ministry activities, I agree that during my participation in, or attendance at, any ministry events, mission trips, or other activities, that I am responsible for my actions.

  • I recognize that everyone in the group is a part of the body of Christ. I will make every effort to ensure that everyone feels welcome and important.
  • Participate fully in all scheduled group activities and abide by additional group guidelines at any ministry events.
  • Follow all instructions given by group leaders and chaperones without complaint.
  • I will respect the physical and emotional well-being of others by treating them as I would like to be treated. (This includes refraining from harsh play or violence, refraining from harmful jokes, respecting the need for personal space, etc.)
  • Respect the health of my own body by refraining from the use of tobacco, alcohol, and illegal drugs. I understand that the use of these substances is absolutely prohibited and could result in disciplinary action.
  • Respect the things I use and the property of places I visit. The areas used for all events, including transportation, shall be left clean.
  • Avoid inappropriate public displays of affection.
  • Stay within the group or assigned sub-group at all times. I will not wander off alone or leave the activity site unless granted permission by an adult, and I will report for all designated check-in times.
  • Hold safety in the highest regard and refrain from compromising my own safety or another’s safety.
  • Provide a trusting environment for my peers. When others share something about themselves in a group discussion, I will not repeat that information to other friends outside of the group.

I voluntarily pledge to follow the Code of Conduct during my participation in all Covenant Church events. I understand that if I fail to abide by this Code of Conduct, I may not be allowed to continue to participate in Family Ministry events.
